Antecedentes La pandemia de COVID-19 obligó a tomar medidas que implicaban la desatención a los pacientes con diabetes tipo 2 (DM2). Objetivos Explorar la repercusión de la discontinuidad asistencial sobre los pacientes con DM2. Diseño Estudio observacional retrospectivo multicéntrico. Emplazamiento Cinco centros de atención primaria (AP), que no tenían protocolo de actuación específica para ellos, durante 2020 y 2021. Participantes Pacientes con DM2 en Tenerife, Islas Canarias, España. Mediciones principales De las historias clínicas se extrajeron el sexo y la edad, las variables de seguimiento del programa de detección y control de la enfermedad vascular ateroesclerótica (pEVA), de cumplimiento de los objetivos de control y frecuentación al médico de familia y enfermera comunitaria. Resultados Se incluyó a 3.543 pacientes, 1.772 (50%) mujeres, de ellos 2.204 (62%) mayores de 65 años. La gran mayoría de actividades registradas y objetivos de control disminuyeron en 2020, recuperándose en 2021 sin alcanzar los niveles de 2019. En 2020 aumentaron las consultas telefónicas y disminuyeron las presenciales, tendencia mantenida en 2021 para las telefónicas. Las mujeres y los mayores de 65 años presentaron mayor frecuentación, más registros de actividades y logros de objetivos de control en la mayoría de los parámetros. Conclusiones La pandemia supuso una sobrecarga de la AP que ha afectado a la atención de los pacientes con DM2, que no ha logrado restablecerse a los niveles prepandémicos. Los hombres jóvenes conforman la diana de priorización de esta atención. Las medidas antipandémicas han aumentado la consulta telefónica, un recurso que debe potenciarse (AU)

Individuals with Alzheimer Disease who develop psychotic symptoms (AD + P) experience more rapid cognitive decline and have reduced indices of synaptic integrity relative to those without psychosis (AD-P). We sought to determine whether the postsynaptic density (PSD) proteome is altered in AD + P relative to AD-P, analyzing PSDs from dorsolateral prefrontal cortex of AD + P, AD-P, and a reference group of cognitively normal elderly subjects. The PSD proteome of AD + P showed a global shift towards lower levels of all proteins relative to AD-P, enriched for kinases, proteins regulating Rho GTPases, and other regulators of the actin cytoskeleton. We computationally identified potential novel therapies predicted to reverse the PSD protein signature of AD + P. Five days of administration of one of these drugs, the C-C Motif Chemokine Receptor 5 inhibitor, maraviroc, led to a net reversal of the PSD protein signature in adult mice, nominating it as a novel potential treatment for AD + P.

BACKGROUND: Lack of external validation of dementia risk tools is a major limitation for generalizability and translatability of prediction scores in clinical practice and research. OBJECTIVES: We aimed to validate a new dementia prediction risk tool called CogDrisk and a version, CogDrisk-AD for predicting Alzheimer's disease (AD) using cohort studies. DESIGN, SETTING, PARTICIPANTS AND MEASUREMENTS: Four cohort studies were identified that included majority of the dementia risk factors from the CogDrisk tool. Participants who were free of dementia at baseline were included. The predictors were component variables in the CogDrisk tool that include self-reported demographics, medical risk factors and lifestyle habits. Risk scores for Any Dementia and AD were computed and Area Under the Curve (AUC) was assessed. To examine modifiable risk factors for dementia, the CogDrisk tool was tested by excluding age and sex estimates from the model. RESULTS: The performance of the tool varied between studies. The overall AUC and 95% CI for predicting dementia was 0.77 (0.57, 0.97) for the Swedish National study on Aging and Care in Kungsholmen, 0.76 (0.70, 0.83) for the Health and Retirement Study - Aging, Demographics and Memory Study, 0.70 (0.67,0.72) for the Cardiovascular Health Study Cognition Study, and 0.66 (0.62,0.70) for the Rush Memory and Aging Project. CONCLUSIONS: The CogDrisk and CogDrisk-AD performed well in the four studies. Overall, this tool can be used to assess individualized risk factors of dementia and AD in various population settings.

Y chromosomal short tandem repeats (Y-STRs) are used in forensic investigations as a useful complementary tool to autosomal markers. The ongoing development of new kits with an increasing number of markers makes it necessary to update populations typed in the Y-STR Haplotype Reference Database to reach at least 23 Y-STRs. A novel Y-STR multiplex panel was developed to offer a cost-efficient alternative to update Y-STR haplotypes from 12 to 23 loci. This panel includes the eleven markers, DYS448, DYS456, DYS458, DYS635, Y-GATA H4, DYS576, DYS481, DYS549, DYS533, DYS570 and DYS643, as well as DYS385a/b for traceability purpose. Developmental validation of this panel was conducted following the recommendations of the Scientific Working Group on DNA Analysis Methods (SWGDAM), showing high sensitivity, tolerance to common inhibitors as well as high species specificity. It was efficient for degraded DNA samples and for detection of male mixtures. When applying it for extending the current data of the Ibiza population, both the discrimination capacity and the haplotype diversity increased from 0.5952 to 0.9048 and from 0.9808 to 0.9977, respectively. Together, the study demonstrates the suitability of this panel in forensic casework.

Resumen El uso de escalas de predicción clínica puede incrementar la detección temprana de enfermedad pulmonar obstructiva crónica (EPOC). Su rendimiento en población latinoamericana ha sido pobremente estudiado. Nuestro objetivo fue determinar la validez y reproducibilidad del cuestionario PUMA, como herramienta de tamización en atención primaria en población colombiana, mediante un estudio tipo corte transversal; donde se establecieron las características operativas del cuestionario, área bajo la curva de características operativas del receptor (ACOR) y el mejor punto de corte para esta población. 1.980 sujetos fueron incluidos en el análisis. La prevalencia de EPOC correspondió a 18,9%. La capacidad discriminatoria del cuestionario fue de 0,69 (IC95%: 0,66-0,72), para un punto de corte óptimo mayor de 5, con una sensibilidad del 60%, especificidad 66% y un valor predictivo negativo de 88%. La escala PUMA para tamizaje de pacientes en riesgo de EPOC tiene una capacidad discriminatoria moderada y una excelente reproducibilidad en la población estudiada.


The use of clinical prediction scales may increase the early detection of chronic obstructive pulmonary disease (COPD). The performance characteristics of these scales in the Latin American population is poorly studied. We aimed to evaluate validity and reproducibility of PUMA questionnaire as a screening tool in primary care in a Colombian population. A cross-sectional study was performed. Operational characteristics of the questionnaire, the area under the received operator curve (AUROC), and the best cut-off point of the score were calculated. 1,980 individuals were included in this analysis. Prevalence of COPD was 18.9%. AUROC of the questionary was 0.69 (CI95%: 0.66-0.72), with an optimal cut-off point greater than 5 (sensitivity 60%, specificity 66%); predictive negative value was 88%. PUMA's scale for the screening of patients at risk of COPD has a moderate accuracy and an excellent reproducibility in the studied population.

Ultrastable lasers are essential tools in optical frequency metrology enabling unprecedented measurement precision that impacts on fields such as atomic timekeeping, tests of fundamental physics, and geodesy. To characterise an ultrastable laser it needs to be compared with a laser of similar performance, but a suitable system may not be available locally. Here, we report a comparison of two geographically separated lasers, over the longest ever reported metrological optical fibre link network, measuring 2220 km in length, at a state-of-the-art fractional-frequency instability of 7 × 10-17 for averaging times between 30 s and 200 s. The measurements also allow the short-term instability of the complete optical fibre link network to be directly observed without using a loop-back fibre. Based on the characterisation of the noise in the lasers and optical fibre link network over different timescales, we investigate the potential for disseminating ultrastable light to improve the performance of remote optical clocks.

Late-life depression (LLD) is associated with an increased risk of developing dementia; however, it is not known whether individuals with a history of LLD exhibit a more rapid rate of cognitive decline. We aimed to determine whether those with LLD experienced faster cognitive decline compared with never-depressed control (NDC) participants from the community and whether stratification of LLD into early-onset depression (EOD) and late-onset depression (LOD) subtypes revealed differing rates and domain-specific expression of cognitive decline. We conducted a prospective, longitudinal study where 185 participants with LLD (remitted) and 114 NDC were followed for 5 years on average. EOD was defined as having first lifetime depressive episode at <60years and LOD at ≥60years. Every year, participants underwent comprehensive neuropsychological assessment. Composite scores for each cognitive domain were calculated through averaging standardized scores across tests. LLD compared to NDC demonstrated significant baseline impairment but did not decline more rapidly. EOD were significantly impaired in attention/processing speed and global cognitive function at baseline but did not experience more rapid decline as compared to NDC. Those with LOD compared to both NDC and EOD performed worse in all domains at baseline and experienced more rapid decline in verbal skills and delayed memory ability. Our findings suggest that baseline impairment may lower the threshold for those with LLD to develop dementia. EOD and LOD may represent distinct phenotypes of cognitive impairment with differing neural substrates. LOD may represent a distinct phenotype with a more rapid decline in verbal skills and delayed memory.

INTRODUCTION: Reconstruction of the anterior cruciate ligament attempts to reproduce the native ligament; make independent tunnels optimize the entry points and effectiveness the anatomy. Perform radiographic measurements of the knee, identify position of femoral and tibial tunnels in patients operated for reconstruction of anterior cruciate ligament with anatomical technique. MATERIAL AND METHODS: Patients were identified in the period January 2018 to December 2019. 98 patients were retrospectively evaluated. Radiographic measurements in the femur, the inclination angle of the tunnel, the insertion site with respect to the Blumensaat line and the transosseous distance were determined. In tibia, the location in percentage of the tibial plateau location in anteroposterior and lateral view, tibial tunnel angle. RESULTS: Postoperative patients were 98. (75.5%) male, female (24.5%). Postoperative in 2018 (57.15%) and 2019 (42.85%) Age range: affected group was 21-25 years, right side affected in 57.15%; Radiographic measurements in femur, the angle of inclination of the tunnel was 45o, percentage in the Blumensaat line was 20%, and the transosseous distance was 3.43 cm. In tibia the percentage distance in anteroposterior projection was 44%, and lateral 28%, The angle in tibial tunnel anteroposterior projection of 73 degrees, and lateral 114. CONCLUSION: The anatomical technique is a good option to perform tunnels with an optimal anatomical position. By making independent tunnels it allows to improve coverage of normal antomic footprint. Increases degrees and inclination of tunnels, causes added injuries.


INTRODUCCIÓN: En la reconstrucción de ligamento cruzado anterior se intenta reproducir ligamento nativo, realizar túneles independientes, optimizar los puntos de entrada y la efectividad de la anatomía; hacer mediciones radiográficas de rodilla, identificar la posición del túnel femoral y tibial en pacientes operados de reconstrucción de ligamento cruzado anterior con técnica anatómica. MATERIAL Y MÉTODOS: Se identificaron pacientes en el período de Enero de 2018 a Diciembre de 2019. Se evaluaron de manera retrospectiva 98 pacientes. Mediciones radiográficas en fémur: se determinó el ángulo de inclinación del túnel, el sitio de inserción con respecto a la línea de Blumensaat y la distancia transósea. En tibia: la ubicación en porcentaje de la localización de platillo tibial en proyección anteroposterior y lateral, ángulo de túnel tibial. RESULTADOS: Los pacientes postoperados fueron 98, masculinos (75.5%) y femeninos (24.5%). Postoperados en el año 2018 (57.15%) y en 2019 (42.85%). Rango de edad: el grupo afectado fue de 21-25 años, lado afectado: derecho en 57.15%. Mediciones radiográficas en fémur: el ángulo de inclinación del túnel fue de 45o, el porcentaje en la línea de Blumensaat fue de 20% y la distancia transósea fue 3.43 cm. En tibia la distancia porcentual en proyección anteroposterior fue de 44% y lateral de 28%. El ángulo en túnel tibial en proyección anteroposterior fue de 73º y lateral de 114º. CONCLUSIÓN: La técnica anatómica es una buena opción para efectuar túneles con posición anatómica óptima. Realizar túneles independientes permite mejorar la cobertura de la huella anatómica normal, aumenta los grados y la inclinación de túneles y provoca lesiones agregadas.

We assessed the effects of ultra dry-air plasma surface treatments on the properties of Al2O3-GPTMS-PMMA hybrid dielectric layers for applications to high-performance amorphous Indium Gallium Zinc Oxide (a-IGZO) thin film transistors (TFTs). The hybrid layers were deposited by an easy dip coating sol-gel process at low temperature and then treated with dry-air plasma at 1, 2 and 3 consecutive cycles. Their properties were analyzed as a function of the number of plasma cycles and contrasted with those of the untreated ones. The dielectric characteristics of the hybrid layers were determined from I-V and C-f measurements performed on metal-insulator-metal and metal-insulator-semiconductor devices. The results show that the plasma treatments increase the surface energy and wettability of the hybrid films. There is also a reduction of the OH groups and oxygen vacancies in the hybrid network improving the dielectric properties. The incorporation of nitrogen into the hybrid films surface is also observed. The plasma-treated hybrid dielectric layers were applied as dielectric gate in the fabrication of a-IGZO TFTs. The best electrical performance of the fabricated TFTs was achieved with the 3 cycles plasma-treated hybrid dielectric gate, showing high mobility, 29.3 cm2 V-1 s-1, low threshold voltage, 2.9 V, high I ON/OFF current ratio, 106, and low subthreshold swing of 0.42 V dec-1.

Además del sistema ABO, los subgrupos del mismo revisten gran importancia en inmunohematología, Los subgrupos A difieren tanto en el número de sitios antigénicos como en la configuración del antígeno eritrocitario. Los principales, A1 y A2 se diferencian en que los eritrocitos A1 son aglutinados por el anticuerpo Anti-A1 humano o por la Lectina Anti-A1 (Dolichos biflorus), y los eritrocitos A2 son aglutinados por la Lectina Anti-H (Ulex europaeus). MATERIALES Y MÉTODOS: se realizó un estudio descriptivo, de Corte Transversal, Se analizó los registros tanto físico y electrónico del Banco de Sangre, se incluyeron todos los donadores efectivos, mismos que fueron tipificados por el laboratorio de inmunohematología en el periodo de mayo a julio del 2018. Método empleado, aglutinación en tubo y en micro placa. RESULTADOS: en un total de 1599 donantes, se determinó que el grupo O tiene mayor frecuencia con un 84% y el menos frecuente fue el AB con un 0,66%. Según el grupo sanguíneo A y AB tenemos las siguientes frecuencias: A1 que representa el (73.3%), A2 el (15.9%), Aint el (5.65%), A1 B el (3.60%) y A2 B el (1.55%). La importancia clínica se basa en que algunas personas del grupo A2 transfundidas con A1 , pueden producir Anti-A1 que es un anticuerpo natural irregular activo a 22 ºC, pero en ocasiones está activo a 37ºC causando una reacción transfusional extravascular, por lo que, si no se cuenta con eritrocitos A2 , se recomienda transfundir eritrocitos grupo O.

RESULTS: in a total of 1599 protocols, it was determined that group O has the highest frequency with 84% and the least frequent was the AB with 0.66%. According to blood group A and AB we have the following frequencies: A1 representing (73.3%), A2 (15.9%), Aint (5.65%), A1B (3.60%) and A2B (1.55%). The clinical importance is based on the fact that some people in group A2 transfused with A1, can produce Anti-A1 which is an irregular natural antibody active at 22 ° C but sometimes it is active at 37 °C causing an extravascular transfusion reaction, so if A2 erythrocytes are not available, it is recommended to transfuse group O erythrocytes.

Resumen: La espondiloptosis traumática es una lesión muy rara y grave generalmente causada por traumatismos de alta energía. Consiste en la dislocación anterior o posterior de 100% o más al cuerpo vertebral subyacente, lo que puede generar compresión y lesión total de la médula espinal, produciendo déficit neurológico; este tipo de lesión representa la etapa 4 y 5 de Allen. Caso clínico: Masculino de 50 años quien sufre accidente automovilístico al colisionar contra muro de contención, generándose lesión de tipo hiperextensión-compresión cervicotorácica, manejado con hemicorpectomía C7, discectomía C7-T1, espondilodesis con placa anterior (C6-C7, C7-T1), toma y aplicación de injerto, abordaje posterior + fascetectomías de C7 + fijación transfacetaria C6 y transpedicular de T1. Discusión: Encontramos que la estabilización temprana con pinza de Gardner más el abordaje anterior y posterior brindan adecuados resultados en cuanto a integridad sensitiva y motora del paciente así como una pronta rehabilitación.


Abstract: Traumatic spondyloptosis is a serious injury usually caused by high-energy trauma; It consists of the anterior or posterior dislocation of 100% or more of the underlying vertebral body, which can become a total injury of the spinal cord, producing a neurological deficit; this type of injury represents stage 4 and 5 of Allen-Ferguson. Clinical case: A 50-year-old man who suffers a car accident, he receive frontal impact when he was a driver, colliding with the retaining wall, referred from another hospital to emergency room, managed with C7 hemicorpectomy, c7-t1 discectomy, spondylodesis with anterior plate (C6-T1), and posterior approach + Fascetectomies of C7-T1, facet joint screws C6 and transpedicular fixation of T1. Discussion: Subaxial cervical spondyloptosis is relatively rare clinical entity, a complete clinical examination is important in diagnosis, taking in considerations the injury mechanism. For treatment we have a multiple options, at this case anterior-posterior (360 degrees) treatment it was the better option for Us; however, must be personalized and consider the early rehabilitation of patient.

Resumen Objetivo: Analizar la participación de los residentes de cirugía plástica de Chile en la publicación científica de los últimos 20 años y evaluar su experiencia durante la residencia. Materiales y Método: Revisión de la literatura desde 1998-2018 bajo los términos: Cirugía Plástica, Plastic Surgery y Chile. Se incluyeron aquellos con al menos un autor cirujano plástico con filiación en Chile. Se registró la participación reportada de residentes y analizaron sus autores según su período de residencia y fecha de publicación, agregándolos como residentes no reportados. Se analizó tema, año de publicación y revista. Se aplicó una encuesta a residentes de cirugía plástica y postbecados recientes para conocer la percepción sobre su participación en actividades científicas. Se comparó la participación entre residentes con y sin año de investigación mediante el test exacto de Fisher. Resultados: Predominó la temática reconstructiva (48,2%), en adultos (68,6%) y en centros universitarios (48,7%). La participación reportada de residentes fue de 8,4%, subiendo a 38,2% al ampliarla a los no explicitados como residentes. Los encuestados expusieron la falta de tiempo como principal impedimento a la publicación y participación en congresos. Discusión: La participación en actividades científicas resulta beneficiosa para residentes, sus tutores y la reputación académica de sus centros. La mayoría de los residentes cree que su participación podría haber sido mayor en caso de que se hubiesen dado más facilidades. Conclusiones: La participación de residentes de cirugía plástica se encuentra subreportada. Programas de investigación, tiempos protegidos y mayor tutorización podrían aumentar esta cifra.


Aim: Evalúate the participation of Chilean plastic surgery residents in scientific publication in the last 20 years and assess their experience during residency. Materials and Method: Literature review from 1998-2018 under the terms: Cirugia Plastica AND Plastic Surgery AND Chile. Publications with at least one plastic surgeon author with filiation reported in Chile were considered. Those with reported participation of residents were registered and their authors were also analyzed according to their period of residence and date of publication, adding them as unreported residents. Subjects, year of publication and journals were analyzed. A survey was applied to plastic surgery residents and recent plastic surgery graduates to evaluate the perception of their participation in scientific activities. Residents participation with and without a previous research fellow was compared using Fisher's exact test. Results: Reconstructive themed studies (48.2%), in adults (68.6%) and in university centers (48.7%) prevailed among the included articles. The reported participation of residents was 8.4%, which rised to 38.2% when it was extended to those not explicitly reported as residents among the authors. Residents exposed the lack of time as the main barrier to publication and congress participations. Discussion: Participation in scientific activities is beneficial for residents, their mentors and the academic reputation of their centers. The majority of residents believe that their participation could have been greater if more facilities had been given. Conclusions: Participation of plastic surgery residents in scientific publications is under reported. The implementation of research programs, protected times and active mentoring could increase this number.

INTRODUCCIÓN Y OBJETIVO Una enfermedad nueva, COVID-19, está afectando dramáticamente al mundo. Conocer los riesgos para la salud reproductiva es un imperativo para la práctica obstétrica y ginecológica. Esta investigación analiza los riesgos maternos y perinatales asociados a COVID-19, con el objetivo de identificar desafíos que la enfermedad plantea a la práctica de la matronería. MÉTODOS Revisión narrativa. Se consultaron artículos científicos de fuentes primarias indexados en las bases Scielo, Pubmed, Scope, WOS, mediante los siguientes términos de búsqueda: "embarazo" "transmisión vertical" "salud materna y perinatal", "riesgos maternos y perinatales" "lactancia materna", COVID-19", "Coronavirus". Se realizaron 3 fases de selección. Los tópicos de análisis fueron: Transmisión vertical, Riesgo materno y perinatal, Lactancia materna. RESULTADOS. En mujeres embarazadas las formas severas de COVID-19 se presentan en presencia de enfermedades crónicas. A nivel perinatal el riesgo mayor es el parto prematuro, generalmente por indicación médica y por cesárea. Aunque no hay evidencias de transmisión vertical, tampoco puede descartarse. Los riesgos neonatales se relacionan con el contagio por proximidad y con medidas restrictivas que pueden afectar la lactancia materna y la interacción madre-hija(o). CONCLUSIONES. La COVID-19 aporta varios desafíos para la práctica de la matronería: implementación de métodos de prevención del contagio a la gestante y a su entorno cercano; adecuación de la preparación al parto en caso de positividad; prevención del estrés y desgaste emocional materno desde el inicio de la gestación hasta el postparto; adecuación de cuidados al recién nacido; investigación aplicada en Latinoamérica, y evaluación de nuevos protocolos.


INTRODUCTION AND OBJECTIVE A new disease, COVID-19, is dramatically affecting the world. Knowing the risks for the reproductive health is an imperative for the obstetric and gynecological practice. This research analyzes the maternal and perinatal risks associated with COVID-19, with the aim of identifying challenges that the disease poses to the practice of midwifery. METHODS Narrative review. Scientific articles from primary sources indexed in Scielo, Pubmed, Scope, and WOS, are consulted by using the following search terms: "pregnancy" "vertical transmission" "maternal and perinatal health", "maternal and perinatal risks" "breastfeeding", COVID-19", "Coronavirus". Three selection phases were carried out. The topics of analysis were vertical transmission, maternal and perinatal risk, breastfeeding. RESULTS In pregnant women severe forms of COVID-19 occur in the presence of chronic diseases. At the perinatal level, the biggest risk is premature delivery, generally for medical indications and by cesarean section. Although there is no evidence of vertical transmission, it cannot be ruled out either. Neonatal risks are related to transmission by proximity and restrictive measures that may affect breastfeeding and mother-child interaction. CONCLUSIONS COVID-19 brings several challenges to the practice of midwifery: implementation of methods to prevent infection of the pregnant woman and her close environment; adaptation of birth preparation in case of positivity; prevention of maternal stress and emotional distress from the beginning of pregnancy to postpartum; adequacy of care for the newborn; research in Latin America, and evaluation of new protocols.
